Job Description:

The Process Improvement Program Manager Embedded partner to a major Primary Care initiative, which includes both the stabilization and spread across departments and clinic sites. Specific expertise in building, implementing, and stabilizing a Daily Management System (DMS) is critical to the success of this project so a high level of knowledge and proficiency is needed. Will lead, facilitate, manage and support cross-functional teams charged with the (re)designing processes critical to improving access, patient throughput, and improving quality aimed at improving value to our customers. Works in partnership with providers, leadership, staff, other key customers and stakeholders for the development, implementation and ongoing assessment of performance improvement and (re) design initiatives. Support the organization's lean leadership culture by demonstrating, supporting, and coaching others on our Principles and Behaviors. Essential Functions: Coaching to stat sheets and performance boards to support DMS efforts. Responsible for coaching/advising/leading operations partners through A3 thinking designing & implementing major initiatives such as, new delivery models. Manages projects according to the methodology, timeline & budget of the organization. Creates governance structure and change management plans. Supports financial performance & business case development. Facilitates model of care design events crafting a conceptual design at the system level, a functional design at the value stream level, a detailed design at the operational level, and standard work development for operating in the new (re) design. Determines appropriate qualitative and quantitative measures to evaluate progress & successful implementation. Works with business groups to build trust & credibility, understand their concerns and help them understand any project challenges Mentors through PDCA/improvement thinking and care model refinement to drive to operational goals. Tracks and coordinates reports on performance. Communicates status updates & delivery expectations to steering committee and stakeholders. Develops the system for future spreads including road map, systems and structures, to drive both "the what" and "the how" to design a new delivery model. Coaches and develops leaders to become lean thinkers. Acts a content expert in applying lean principles and utilization of lean tools. Provides support and coaching for leaders in the daily management structure, front line problem solving and leader standard work. Leads and acts as internal consultant and catalyst educating and training in the use of continuous improvement process improvement methodologies; promotes and guides the use of appropriate measure tools and methodologies, e.g., Value Stream Mapping, 5 S, standard work, level loading. SKILLS AND ABILITIES: Ability to facilitate and guide the application of lean principles and behaviors. Exceptional written and verbal communication. Comfortable engaging with stakeholders at all levels of the organization, including Provider stakeholders. Ability to lead a cross-functional team while motivating team members to their highest potential. Excellent business presentation skills. Demonstrated success at building team relationships and partnerships across organizational lines. Ability to assess and monitor financial and operational performance, and use this to identify opportunities for improvement. Ability to create and manage an operational plan, develop and manage budgets, and evaluate implementation budget performance to achieve expectations. Excellent analytical skills. Proficient in computer skills, including Microsoft Office Suite software tools.
