Case Manager (Care Management) - Posting #22946

Position Summary:

Full-time position available for the Care Management Program located in Vancouver on Fourth Plain St. We prefer someone that can speak either of these languages: Spanish, Russian, Chuukese, ASL. English is required. If interested, please additional requirements below.

The Interdisciplinary team member is responsible for providing program support, community outreach, and case management for community health center patients receiving care management.

The Care Management Case Manager has a basic understanding of how to help clients who have both physical and mental health needs, and understands how to promote a healthy lifestyle and promote patients ability to choose positive and healthy outcomes. May assist with case management, outreach and engagement, and technical support. We are looking for someone who has the capacity to understand the impact of chronic disease and working with social determinants that impede self-care. There will be additional training after hired.

Education and/or Requirements:

  • High School Graduate, GED or equivalent
  • Must have two years social service work or have done similar work.
  • Experience with MS Outlook and electronic data management.
  • Must have the ability to multi-task.
  • Must be fully vaccinated against COVID-19.
  • Bilingual in English/Spanish, English/Russian, English/Chuukese and ASL preferred but not required.
