Job Description:
This position manages staff who case manage Veterans Grant & Per
Diem transitional housing program and homeless or at risk of
homelessness and provides oversight of the day-to-day operations of
the Veterans Programs. The Veterans Program Manager works closely
with the EAC, VA programs, and other service providers to
coordinate services and ensure ongoing supervision of staff and
program participants as they work on income enhancement, finding
permanent housing, and developing self-sufficiency. The Veterans
Program Manager acts to ensure the stability of staff and the
client within the programs, is familiar with/able to access
resources for benefits and entitlements, alcohol and drug
treatment, mental health treatment, and housing, and is effective
in working independently, as a member of a team, and as a part of a
larger coalition.
RESPONSIBILITIES:
- Provides supervision and support to VGPD staff.
- Provides assistance to staff & clients in working toward the
program goals of income enhancement, seeking permanent housing, and
developing self-sufficiency.
- Assists with staff efforts to connect residents to appropriate
community-based resources and assist with problem-solving issues
with staff and clients.
- Monitors length of stay in the program, including enrollments,
discharges, and any other reporting requirements per
building/program policy.
- Provides client assessment and develops goal plans. Monitor
client progress. Counsel and guide staff and clients in developing
goals.
- Provides ongoing individual support and encouragement to staff
and clients, along with conducting reviews of their progress while
monitoring clients' 90-day plans with staff to ensure plans are
followed and updated.
- Facilitates weekly team meetings and provides ongoing training
to ensure case managers' skills and abilities comply with VA
requirements.
- Complete quarterly reports and annual inspection reports and
submit them to VA Liaison.
- Reviews and approve monthly billing and submit to
accounting.
- Monitors staff schedules and approves/submit time cards.
- Educates clients and staff about employment assistance options,
service/non-service-connected and social security benefits, and
community engagement opportunities.
- Collaborates with community resource agencies to remove
barriers, provide intervention and make referrals for issues facing
customers in a variety of areas including, but not limited to,
employment, substance abuse, health, finances, and legal/criminal
issues.
- Identifies community and individual issues that could result in
a client's status change and work to achieve appropriate
resolutions.
- Works with internal and VA service providers to ensure
coordinated and streamlined services.
- Insures appropriate input and maintenance of accurate records
of customer progress in working toward individualized program goals
using MS Word, Access, and other software.
- Participates in compliance, quality assurance, and quality
improvement activities as directed.
- Represents CCC Supportive Housing in the greater community and
attends HADIN (Homeless Alcohol and Drug Intervention Network),
424, Veterans Task Force, and other community meetings.
- Adhere to all state and federal privacy and security
regulations applicable to the program and CCC policies and
agreements regarding confidentiality, privacy, and security. This
includes immediately reporting any breach of protected health
information or personal identifying information of any person
receiving CCC services by CCC or an outside provider to the CCC
Compliance Department, as well as to your supervisor or their
designee.
QUALIFICATIONS:
- A Bachelor's degree in a human services field and 3 years of
relevant experience with the Veteran population or an equivalent
combination of education and experience is required.
- Must embrace the mission and philosophy of recovery and
client-centered treatment.
- Experience facilitating training, groups, and workshops.
- Will be required to carry a cell phone for work use. Cell phone
to be provided by Central City Concern.
- Must have knowledge of area housing resources, human service
agencies, and community groups.
- Must have necessary computer skills to accomplish duties;
typing skills desired.
- Must possess a current driver's license, pass a DMV background
check, and be designated an "acceptable" driver as outlined in
Central City Concern's Fleet Safety Policy. Must pass an initial
driver training within 60 days of being an approved driver and
continued recertification training.
- Military Veteran with "General under Honorable" or "Honorable"
Discharge preferred.
- Sufficient manual dexterity and physical ability to perform
duties.
- Must pass a pre-employment drug screen, TB test, and criminal
background check.
