Home Health RN / Travel RN - Position: Home Health Registered Nurse
(RN) – Travel Assignment (Homecare Nurse) - Location: Salem, OR
97301 and surrounding areas (up to 75-mile travel radius including
Canby, Molalla, Woodburn, Colton) — ideal for Salem home health
jobs and Oregon nursing candidates - Start Date: March 30, 2026 -
End Date: June 29, 2026 - Contract Length: 13 weeks - Schedule: Day
Shift, Monday–Friday, 8:00 AM – 5:00 PM (5x8-hour days) - Hours per
Week: 40 - Estimated Weekly Pay: $2,050 – $2,160 - Patient Volume:
Average 5–8 patients/day; assigned 5–10 patients/day (productivity
expectation: 30 points/week) - Travel: Up to 75 miles from home
clinic; ~250 miles/week (mileage reimbursed at IRS standard rate
for patient-to-patient travel) - Dress Code: Scrubs - EMR /
Documentation: HomeCare HomeBase (HCHB) and PointCare; OASIS
experience preferred - On-Call/Weekend Rotation: 1 weekend on-call
every 4th week (varies by location; details discussed during
interview) Job Requirements — Home Health RN Qualifications -
Experience: Minimum 1 full year of recent Adult Home Health RN
experience (or 1 year Hospice RN experience, depending on
specialty) - License: Active Oregon RN license (must be in hand at
time of submission; pending licenses not accepted) -
Certifications: Current BLS and CPR required - Documentation
Skills: Proficiency with OASIS and HomeCare HomeBase (HCHB) within
the last 6 months, or willingness to complete retraining during
compliance - Transportation: Reliable vehicle, valid driver’s
license, and auto insurance (daily travel required) - Other: Must
be open to on-call rotations and traveling up to 75 miles from
assigned care center; first-time travelers not accepted; local
candidates accepted at same rate - Productivity: Must meet weekly
productivity points of 30 (equates to 40 hours/week); no guaranteed
hours Responsibilities — Homecare Nurse / Home Health Nursing
Duties - Provide, coordinate, and direct skilled home health
nursing care according to physician orders and agency policies -
Perform Start of Care (SOC), wound care, skilled follow-up visits,
medication management, and other skilled nursing interventions in
the home setting - Complete accurate, timely documentation in
HCHB/PointCare; finalize all visit documentation within 24 hours -
Assess, plan, implement, and evaluate individualized patient care
and OASIS assessments - Educate patients and caregivers on disease
management, medications, safety, and care plans to support
home-based recovery - Supervise and evaluate Home Health Aides and
LPNs/LVNs as applicable - Meet productivity expectations of 30
points per week (equivalent to 40 hours/week) - Participate in
interdisciplinary collaboration and periodic office meetings;
adhere to agency compliance, safety, and quality standards Why
