Summary: Under the supervision of a Registered Nurse or Clinician,
the Licensed Practical Nurse (LPN) in the respite care setting
delivers high-quality, culturally appropriate, and patient-centered
care. This role involves providing transitional care for patients
recovering from acute illnesses or injuries, ensuring continuity of
care, and addressing medical, psychosocial, and recovery needs,
promoting health stability and continuity of care. The LPN
contributes to planning, implementing, and evaluating care plans
while adhering to organizational and regulatory standards. Key
Responsibilities: 1. Patient Care & Flow: Provide direct,
age-specific care to patients, including administering medications
and treatments as prescribed. Monitor and document patient vitals,
growth and development needs, and deviations from norms, reporting
findings to supervising RN or clinician. Coordinate room turnover,
assess patient ambulation, and activities of daily living (ADLs).
Perform venipuncture and simple lab testing (e.g., urine dip,
hemoccult, glucose monitoring). 2. Care Coordination: Collaborate
with interdisciplinary teams to develop and execute individualized
care plans. Track and document critical follow-ups using EMR and
other electronic systems. Assist patients with scheduling and
accessing ancillary services, such as lab work and specialty care.
Facilitate care transitions, including discharge planning. 3.
Health Education: Teach patients and families about self-care
practices, medication adherence, and chronic disease management.
Deliver health education in group or one-on-one settings. 4.
Emergency Response: Recognize and respond to medical emergencies,
such as hypoglycemia, following CPR and emergency triage protocols.
Contact supervising RN/clinician as necessary for assistance with
emergent situations. 5. Equipment & Supplies: Clean and prepare
medical equipment for sterilization; maintain inventory for patient
exam rooms and work areas. Use and maintain medical devices such as
pulse oximeters, nebulizers, glucometers, and CLIA-waived lab
equipment. 6. Quality Improvement: Participate in quality assurance
and improvement activities as requested. Collaborate in
organizational initiatives to optimize care delivery. 7. HIPAA
Compliance: Access and use patient health information (PHI) per
minimum necessary guidelines to schedule appointments, answer
medical inquiries, and coordinate care. 8. Administrative Support:
Assist with patient scheduling, filing, and other administrative
duties as needed. Respond to patient telephone inquiries, including
triage and urgent care needs, under supervision. 9. Professional
Development & Collaboration: Engage in staff training, case
conferences, and professional development opportunities. Contribute

Excellent organizational skills. Education and Experience: Current
State of Oregon Licensed Practical Nurse License and Current CPR
(health care provider level) 1 year experience as a Licensed
Practical Nurse, preferred. Proficiency in English and Spanish,
both written and spoken language, preferred. Previous work in
respite care, transitional care, or community health preferred.

Physical Requirements: Standing: 20% Walking: 25% Sitting: 50%
Lifting/Carrying: Up to 40 lbs. Working Conditions: Work in
well-lit, ventilated environments. Exposure to bloodborne pathogens
and hazardous chemicals. Equipment Used: Medical devices: EKG,
glucometer, nebulizer, autoclave, and others. Office equipment:
Computer, telephone, fax, and copier. Immunization: Staff members
must meet immunization requirements as stated in VGMHC’s
immunization policy and state and federal guidelines. Job
