Registered Nurse (RN) | Home Health Location: Portland, OR Agency: Cynet Health Pay: $2,499 per week Shift Information: Days - 5 days x 8 hours Contract Duration: 13 Weeks Start Date: 3/9/2026 About the Position Job Title: Registered Nurse Profession: Nursing Specialty: Home Health Duration: 13 weeks Shift: Day Hours per Shift: 8 Experience: Minimum 1 full year Adult Home Health Experience or 1 full year Hospice experience License: Must have license in hand Certifications: CPR, BLS Must-Have: Proficient in HomeCare HomeBase (HCHB) and Oasis Description: Provides coordinates and directs the provision of home nursing care according to physician's orders based on agency policies and procedures through the competent application of the nursing process. Responsible to the Clinical Manager. Directly supervises and evaluates Home Health Aides and Licensed Practical Nurses. Home visits and periodic office meetings are required. Must have reliable transportation and current driver's license. Agency-required liability insurance is necessary. Average number of patients per day is 5-8. Contractor will be assigned between 5-10 patients per day. Each type of visit and activity has a different point value. No guaranteed hours are provided. If a situation arises where the contractor cannot meet financial needs due to low patient census, please reach out to discuss floating to a nearby clinic for additional hours. Types of cases include Start of Care, Wound Care, and follow-up visits. First-timers are not accepted; recent specialty experience is required. Weekend requirements vary per location and should be discussed during the interview. On-call requirements will vary. Must be willing to travel up to 75 miles from the assigned care center. Mileage reimbursed at the IRS standard rate. Travelers are compensated from patient home to patient home. Commute to and from the first and last patient is not billable. Local candidates are accepted at the same rate. Must be proficient in HomeCare HomeBase (HCHB) and Oasis. Documentation must be completed within 24 hours of the appointment with the patient. Delayed and incomplete documentation may impact billing and patient care. Clinician must hit weekly productivity points of 30 which equates to 40 hours per week. All overtime must be pre-approved. All travelers must be proficient in the required EMR systems. Must be open to on-call rotations. Home Health Checklist is required. Expected weekly travel distance is between 20-40 miles. Expected number of weekly units of productivity is 30. The clinician will be required to accept all patients on their schedule regardless of distance. Requirements Required for Onboarding BLS
